New City's Jawonio Receives $350,000 Grant For Coordinated Care Services NEW CITY, N.Y. -- State Sen. David Carlucci (D-Rockland/Westchester) announced a $350,000 grant to fund the expansion of coordinated care services at the Health Center at Jawonio, 260 North Little Tor Road, New City, on Nov. 10, at 1 p.m. The funding will help Jawonio assist those whose health needs require both medical and behavioral health services. Jawonio is planning to embed both a full time care coordinator and outreach worker into the health center.
